I am a journalist interested in issues around avoidable baby deaths, and I am always looking to speak to couples who have been affected by avoidable hospital baby loss. This is where a baby died as a result of something in a hospital setting that should not have happened but did, because of human error.

While the overwhelming experience of baby birth in Ireland is a positive one, as evidenced by the number of babies born alive and thriving, human error does happen. But because the same errors happen over and over again, I am interested in writing about the small - relatively speaking - proportion of maternity journeys that end tragically in circumstances that simply should not have happened had lessons been learned and shared right across the board.

Hundreds of babies have died in Ireland since 2013 who should not have died and their voices deserve to be heard through the actions of parents who want things to change so nobody else has to go through what they have gone through.
